Mucocele — The Lump on Your Lip That Comes and Goes
A mucocele is a soft, bluish, dome-shaped swelling caused by leaked saliva from a damaged minor salivary gland duct, most commonly on the lower lip after lip biting. It often bursts, deflates, and refills over weeks. Conservative removal of the involved minor gland is the only durable cure; simple incision recurs.

Why incision alone fails
Simply popping or incising a mucocele drains the saliva but leaves the source gland intact. The gland refills the cavity and the mucocele recurs. Definitive treatment removes the lesion together with the feeding gland.
Treatment
Small mucoceles can be observed; persistent or recurrent ones need conservative surgical excision under local anaesthetic. Floor-of-mouth ranulas may need marsupialisation or partial gland removal.
Ranula — the floor-of-mouth variant
A ranula is a mucocele of the sublingual gland, presenting as a bluish dome on the floor of the mouth. A 'plunging' ranula extends through the mylohyoid muscle into the neck and presents as a soft cervical swelling. Definitive treatment usually requires partial or complete sublingual gland excision, not simple marsupialisation.
Preventing recurrence after excision
The single biggest preventable cause of recurrence is the original habit — chronic lip biting. Awareness training, occlusal adjustments where a sharp tooth is responsible, and short-term use of a soft mouthguard during sleep all reduce recurrence.
When the lump is not a mucocele
A firm, fixed, non-fluctuant lip nodule that does not deflate is a different lesion — minor salivary gland tumour, fibroma, or lipoma — and needs excision with histopathology. Do not assume every lip lump is a mucocele.
